When you complete a patient intake form, request an appointment, or otherwise make an inquiry on our Site, in addition to your Contact Information, we collect any other information you provide in connection with your inquiry, which may include sensitive information related to your physical and mental health, medical history, current medications (including both prescription and non-prescription), family medical history, and general health condition. As part of the intake process, we may also collect certain personal information, including your height, weight, age, and date of birth.
We understand that this health information is sensitive and take measures to safeguard such information as set forth herein.

Usage Information

Payment/Transaction Information
When you enroll as a member of our practice or make a payment, we collect your name, address, and transaction details. Payment card information (credit card number, security code, and expiration date) is collected and processed by our third-party payment processor. We do not store your full payment card information on our systems. Your use of the payment processor’s services is subject to that processor’s own privacy policy and terms of use. We also keep a record of your transactions and membership payments.

What are first and third-party cookies? Health Information.We collect sensitive health and medical information to provide our services to you. No Profiling.We do not use personal information for purposes of profiling in furtherance of decisions that produce legal or similarly significant effects concerning the individual. Under Maryland law, “profiling” means automated processing performed on personal information to evaluate, analyze, or predict personal aspects related to an identified or identifiable individual’s economic situation, health, demographic characteristics, personal preferences, interests, reliability, behavior, location, or movements.
No Sales of Personal Information.We do not sell personal information to third parties as defined under applicable law.
Targeted Advertising.We do not use personal information of consumers for purposes of targeted advertising as defined under applicable law.

Corporate Transactions

Authentication Process. We are required to verify the identities of those who submit requests to exercise certain of the above rights. As a part of this process, you will be required to provide your name, email address, mailing address, and/or telephone number. If you use an authorized agent to submit a request, we may also require the authorized agent to provide proof that you gave the agent permission to submit the request. If we cannot authenticate a request, we will provide notice to the individual that we cannot authenticate the request until the individual provides additional information reasonably necessary to authenticate the individual and their request.
Appeals. Maryland residents have the right to appeal a refusal to take action on a request within a reasonable time after your receipt of the decision.
